My Father passed away last month due to medical negligence by wrong usage of a medical device causing suffocation.
I feel guilty that I could not save him.

On my part, I'm inclined to think your father's suffering started a couple/few months back. Kindly consider sharing the exact date/time of his passing, for purely academic purpose.
