Piyawajiran, the 6th is a dusthana because it causes difficulties such as anxieties, worries, self-doubt, debts, illness, enemies but it is also an upachaya because through it a person can develop skills and techniques, they can practice things, improve themselves, improve their diet, their health, etc. So it is a mix, it brings problems but also offers a lot of opportunities for improvement and growth.
Lord of the 6th is important in determining the difference between harsha yoga and astra yoga. Harsha yoga occurs when the lord is in another dusthana or when it is weak or debilitated, astra yoga occurs when the lord is strong in a good house. In my opinion, harsha yoga tends to make a person less inclined to harm or hurt others while astra yoga (described as 'rough and arrogant') makes a person more willing to fight or argue (i.e. defeat enemies). Strong 6th can give more troubles with enemies and debts, while a weaker 6th is not as problematic. With health, I think malefics in the 6th are the best thing overall for a strong constitution regardless of where the ruler is.
