Very worried about my Jaw/TMJ Problem - do i need surgery?

Dear Forum,
I'm Kaldesh - and i have Stage 4 TMJ - Jaw problems. I am not sure to whether I need surgery - is there a way to fix this? What has caused this problem with right jaw and why all of sudden its getting worse? Is there any hope?
Otherwise my health is generally fine - but this stupid jaw problem has started since last year.
