how does one become a surgeon, a cardiologist, a pathologist, engineer etc etc.

Please consider the following factors for determining profession of a native.

Planets that aspect 10th house of D1
Lord of 10th in D1
Natural karakas of 10th, Sun(status & fame), Merc(professional ability), Jupiter(growth & prosperity) and Saturn(actions)
10th lord and its dispositor in D9
Artha trikona - 2nd, 6th and 10th house in D1and D10
D10

Comparative strength of 6th and 7th house in D1 would reveal if job is opted for or business is undertaken.
Nearly all medical professions need a strong cancer(healing) and scorpio(medicine).
Surgeon - Mars (drugs) or Ketu(forceps) or Mercury(skin & cuts) have to be strong and connected with 10th/10L.
Engineers - In itself is a vast field, given the 21st century context. Nodes are of tremendous importance in engineering career. If its more of field work, involving structures, use of tools like in civil and mechanical engineering - Saturn's influence is responsible. If use of tools turns more refined and it becomes table-work, Venus is responsible for it.
