Dear SNR
Since Rahu & Ketu are always 180 deg apart, some of the time they are in each other's nakshatras.
But there is no special signification to that.
They are more likely to give results as per their sign dispositor, planets aspecting & conjunct them.

Yes, exchange is seen in all D-charts.
It is believed that planets in exchange give each other's results in their AD/PD
