Twins Birth charts and their life

My grandfather and his brother are identical twins. when these 2 babys born there wont be too much time lapse.

so obviously their birth chart should resemble the same confuguration of ascendent, planetary position.

My Grand father died at the age of 52 due to liver problem. where as his brother is still alive at the age of 87.

why do we have this difference when the birth chart configuration is same.
