Grahana in a chart

Dear Learned Jyotishis,

How is a grahana to Sun or Moon determined in a chart ? For normal mundane purposes(Vedic or Tantric Prayogas and for purpose of seeing if there is a dosha by these methods), the grahana is not considered unless it is visible from the location, but this would of course be considered only for Sayana as far as I am aware in Jyotisha.

If Surya/Chandra and Rahu/Ketu are present in the same sign is it considered a grahana affecting the native ? Is there any number of degrees of separation which would need to be considered for this ? What other factors are involved or need to be considered ?
